WILSON PERMISSION/LIABILITY FORM

1. General Participation & Liability Release

I give permission for my child to participate in all regular programs, events, and activities of Wilson Church’s Children’s and Youth Ministries. I understand that reasonable safety precautions will be taken at all times by church staff and volunteers, and I agree to hold harmless and indemnify Wilson Church, its employees, and its volunteers from any and all claims resulting from participation, except in cases of gross negligence or willful misconduct.

In case of emergency, I authorize the ministry leaders to secure any necessary medical treatment for my child and agree to assume responsibility for any related costs.

2. Communication Consent (For middle/high school youth only)

I give permission for approved ministry leaders to communicate directly with my youth through text messages, phone calls, emails, group chat, and direct messaging.

3. Media Release

I give permission for my child’s image (photo, video, or audio recording) to be used by Wilson Memorial Church in church-related communications, including social media, website, printed materials/bulletins/newsletters, and livestream recordings.

4. Transportation Consent

I give permission for my child to be transported by approved ministry leaders for offsite activities.
